• SPORTS PHYSICALS
     
    Any student who wishes to play sports must complete the DIAA Physical Form.
     
    Students in grades 9-12, who have completed the Wellness Center Consent and Health History forms, may have their physicals performed in the Wellness Center.  
     
     
    Students requesting a sports physical are asked to complete a DIAA Sports Physical Form prior to scheduling an appointment.  All areas of the form must be complete with the exception of page 4, the physical examination form. This page will be completed by the medical provider during the physical exam. 
     
    Forms can be picked from the Wellness Center or by accessing the link below. 
     
     
     
     Students not playing a sport and enrolling/transferring will need the State of Delaware Student Health Form (file attached)
     
     
    ** 9th grade and new student physicals can also be completed on the DIAA sports physical form.
