The Wellness Center provides immunizations to any student who is eligible to receive vaccine from the Vaccine for Children Program. No vaccines will be given without additional consent from the Parent. To be eligible you must meet the following criteria:
- Be 18 years of age or Younger (And meet one of the following below)
- Enrolled in Medicaid
- Have No Health Insurance
- American Indian or Alaska Native
The Wellness Center offers the following Vaccines:
- Seasonal Influenza
- Tdap (Tetanus and Pertussis)
- TD (Tetanus)
- Polio
- HPV (Human Papilloma Virus)
- MMR (measles Mumps and Rubella)
- Hepatitis A
- Hepatitis B
- Meningitis (Menactra ACYW)
- Men B
The following link, from the CDC, will provide you with the Vaccine Information Statement (VIS) for each individual vaccine. The VIS will provide you with up to date information about each Vaccine, the risks and benefits.

If your child needs vaccines, feel free to call the wellness center at 302-653-2399 for more information on eligibility or to schedule an appointment. Attached you will find our consent form for vaccines. The signed consent must be returned before any vaccine will be given. Signed consents can be dropped off during regular hours, sent in with your student, emailed to wellness@smyrna.k12.de.us or Faxed to 302-653-1342.
