    NEW REGISTRATIONS

      

    Future Eagles and Parents/Guardians:

     Listed below are the required documents needed to register your student.

     All required documents that are not emailed to the SHS Registration email must be provided in person by the legal parent/guardian before the student(s) can be registered with Smyrna High School.

    Documents to Be Provided

    (Copies will be made and originals will be returned to parent/guardian)

    • Current Photo ID of the parent/guardian
    • Original Birth Certificate
    • Custody/Guardianship Papers (call with questions)
    • Immunization Records
    • Student Physical
    • Registration Packet( see below) 
    •  High School Transcript
    • Most Recent Report Card
    • Withdrawal Grades
    • Discipline Record
    • Attendance Record
    • IEP / 504 Plan  (Special Education Services)
    • Proof of Address (see below)

     ** Please Note: You may have additional paperwork to complete when you arrive at SHS.

     I OWN in the Smyrna School District

    You MUST bring ONE of the following:
     Mortgage Statement, Deed, Sales Agreement or 
     Current Property Tax Bill

     AND 

     ONE of the following:
    Utility Bill (Electric, Gas, Water, Cable)
      

    I RENT in the Smyrna School District

     You MUST bring the following:
    Current signed lease/rental agreement

     AND

     ONE of the following:

    Utility Bill (Electric, Gas, Water, Cable)

    I LIVE with someone who OWNS/RENTS in the Smyrna School District
    You MUST complete a Multiple Occupancy form at:
